Application For Apprenticeship

Qualifications necessary for an applicant to be considered for a plumbing apprenticeship:

If you are accepted for a probationary apprenticeship, you will be required to:

1. I must personally complete this application and include all supporting documnets.
2. I can apply only one time this school year.
3. I must take a written evaluation when scheduled or my application will be canceled.
4. It is my responsibility to notify the J.A. & E.C. AND the Union Office of any change of address. The J.A. & E.C. is not responsible for fialing to notify me if I do not have a current address on file.
5. I must submit either a high school diploma or GED Certificate along with High School Transcripts or GED test scores.
6. I must submit a birth certificate.
7. Before any consideration can be given for additionmal schooling, a transcript must be submitted.
8. A DD 214 must be submitted by all applicants who served in the military.
9. I understand that any false statements made on the application, or any alteration of supporting documents will void my application or cancel my Apprenticeship (if accepted). I further understand that all apprenticeship textbooks are included, however I may have to purchase textbooks for advanced certifications.
